When I first began considering entrepreneurship as an advanced practice nurse, I knew I would only pursue those areas of healthcare that I am truly passionate about and that align with my personal and professional core values. I also knew that whatever opportunities I pursued needed to be focused on making healthcare better.
That’s when I learned about Chronic Care Management.
The premise of Chronic Care Management is to proactively address patient needs to improve patient outcomes. As an emergency department nurse and later a hospitalist nurse practitioner, I was quite familiar with the more reactive approach - and in-turn had witnessed overutilization of the emergency department and potentially preventable hospitalizations. As a patient myself, I have personally experienced lack of coordinated care in the disjointed U.S. healthcare system, despite having a phenomenal medical team. I have had to advocate for myself and family members who have been patients - and then have wondered, “Who is advocating for patients who don’t have a family member in healthcare?”
Chronic Care Management (CCM), a Centers for Medicare and Medicaid Services (CMS) program, is something I strongly believe makes a difference in the lives of patients and in the healthcare system when implemented and operated correctly. Since founding MedAlign Innovative Solutions and our nurse-driven CCM outsourcing agency, I have had the opportunity to lead a team of nurses who are passionate about connecting patients with resources, providing practical education, and supporting patients and their caregivers in-between clinic visits. This proactive approach has resulted in positive patient outcomes - better control of diabetes, improved medication adherence, increased community engagement, reduction of caregiver burnout, and more connected care - all while generating revenue for our clients and decreasing overall costs for the healthcare system. It’s a win-win-win for patients, healthcare organizations, and taxpayers.
